Initial Required Information

You can start your claim now and then upload additional documents later. Depending on the type of claim filed, you will likely need the following information to initiate a claim:

  • Property management company name and reservation number
  • For Trip Cancellation, Trip Interruption and Trip Delay claims, the address of the property where you stayed
  • Personal information (contact info, birthdate, etc.)
  • Travel information (scheduled checkin date, scheduled checkout date, etc.)
  • Details surrounding the incident or loss (date of occurrence, description of situation, etc.)

A Note About Claims Related to Medical Situations:

If you are filing a medical expense claim or a trip cancellation or trip interruption claim that is due to a medical situation, you will need to obtain a statement from the patient’s treating physician.

This is not required to begin the claim process. You can file your claim now and upload the Attending Physician Statement form once it is completed.
